Migrating a WordPress site with data and plugins always goes wrong, how can I migrate it all at once?

I recently migrated an old WordPress site to a new server with a different domain name and environment. I ran into a couple problems with the migration:
Some articles are missing after database import
Some plugins report errors or are not enabled when migrated over
There are a lot of incorrect image paths and internal links that need to be manually changed
