I need to replace the damaged index.php, how can I do it?

I replace damaged index.php file, it all automatically reverts to the tampered version. I've re-uploaded all WordPress core files (except the wp-config.php,uploads directory and the current topic), but the problem persists. This may be due to hidden malicious code, how do I find out the source of the malicious code and remove it completely?

  First, make a full backup of your website files and database, and check index.php for tampering, mainly checking /wp-content/uploads/ /wp-includes/ /wp-content/themes/{your-theme}/ /wp-content/plugins/ /tmp/ /wp-config.php (sometimes tampered with), find it and replace all the core WordPress files
